No, ROYCE! The outside diameter of this Litens Idler/Tensioner Pulley - Part # 900244A is 2-3/4"-inch.
If this part is only for the Idler pulley, why does the description state Tensioner Pulley?

My 2003 Toyota 4-runner has "3" pulleys ... two of them are the same part ... the lower one is different, so I believe that the pulley name reflected the other two pulleys, as it could be used in either location. I had initially bought the pulley attached to the tensioner assembly, but decided to only change the pulley when I discovered that I would have to remove the bolts for larger items (AC compressor, etc.) to gain access to the mount bolts to change the tensioner. My problem was a squeaky sound when the car was running ... the belt replacement did not fix the problem, but when water was sprayed on the belt, the squeak stopped for a minute or so and then resumed. The pulley came with a spacer, as well, which was a change from the original version ... the problem was corrected with the pulley replacement, and has worked good for the past few months. Hope this helps.

Both parts are the same. One mounts to a stable non-moving bracket, the other to a spring tensioned bracket. The pulley is the same for both applications.

The pulley also replaces the one on the tensioner.

PULLEY IN PICTURE LOOKS LIKE OEM. THE ONE IN BOX I GOT LOOKS LIKE A CHEAP AFTER MARKET.

Hi Adam! I can understand your concern. I apologize for the difference in the part image with the one your received. The part image is only a reference and the actual part may vary from that.
